Chronic Disease Management

Non-communicable diseases (NCD’s) are the cause of 41 million deaths worldwide annually. Children, adults and elderly persons are at risk of NCD’s with contributing factors such as unhealthy diets, physical inactivity, exposure to tobacco smoke and harmful use of alcohol.

The Chronic Disease Management programme assists members with management of their chronic condition and includes both benefit management and clinical advice.

If you have been newly diagnosed or living with a chronic condition, please complete the Chronic Medication Form and submit to managedcaredepartment@afa.co.bw

Where to get your medicine ?

  • Members may choose from over 160 registered pharmacies around the country to collect prescribed chronic medicines. Pulamed does not impose any restrictions when selecting a service provider.  Find A Doctor

Confidentiality

  • We strive to keep all information about our patients and their diagnosis as confidential as possible. Your information will not be disclosed to any third parties, employees or any person unless with your express written consent. Members have the option to personally deliver their Chronic Forms or you can scan and email to managedcaredepartment@afa.co.bw

The Chronic Disease Benefit

  • Covers you for any condition from our defined list of chronic conditions. As a member diagnosed with a chronic illness, you will need to apply to Pulamed to have your medicines covered for your Chronic condition. This means either filling out the Chronic Disease Management Application Form or HIV/ART Application Form

Medicine Cover for Chronic Conditions

  • Members get full cover for approved chronic medicine from our medicines list. For medication not included in the list, Pulamed will cover the member up to a set tariff amount. This simply means that the Fund will pay up to the prescribed amount and members may be liable for out-of-pocket expenses.
