Know Your Customer (KYC) Exercise

Are you KYC Compliant?

In compliance with the Financial Intelligence (Amendment) Act 2019 and its Regulations, Pula Medical Aid Fund (PULA) requests its members to ensure that their KYC is up to date.

What do you need to submit?

For Individuals:

  • Certified copy of valid identification document (Omang for citizens and Passports for non-citizens)
  • Proof of residential address (utility bill, lease agreement, title deed, letter from Kgosi, police affidavit)
  • Proof of source of income (payslip, letter confirming employment, bank statement)
  • Fully completed PULA KYC form (accessible via www.pulamed.co.bw or may be collected from PULA offices at Associated Fund Administrators Botswana (AFA).

For Non-Individuals

  • Incorporation document(s) (Certificate of Incorporation, Certificate of Registration, Deed of Trust, etc)
  • Proof of residential address (utility bill, lease agreement, title deed, affidavit, confirmation letter from Company Secretary, etc)
  • Resolution authorising one to act on behalf of the entity.
  • Authorised signatory’s identification document (Omang for citizens, Passport for non-citizens)
  • Proof of bank account.
  • Proof of income tax registration and/or VAT registration (as applicable).
  • Evidence of the entity’s current shareholding and/or directorship (as applicable).
  • Shareholders’ valid identification document (Omang for citizens, Passport for non-citizens).
  • Directors’ valid identification document (Omang for citizens, Passport for non-citizens).
  • Fully completed PULA KYC form (accessible via www.pulamed.co.bw or may be collected from PULA offices at Associated Fund Administrators Botswana (AFA).

Complete KYC documents must be sent to aml@pulamed.co.bw on or before the 31st of March 2021.
